SUPPORTED INDEPENDENT LIVING
DAILY SUPPORT. GREATER INDEPENDENCE. A HOME THAT WORKS FOR YOU.
At My Own Care (MOC), our Supported Independent Living (SIL) services are designed to help NDIS participants build confidence, develop everyday living skills, and live as independently as possible in a safe and supportive home environment.
We take a person-centred and capacity-building approach, working closely with participants, families, support coordinators, allied health professionals, and other members of the support network where appropriate. Each SIL arrangement is tailored to the participant’s individual needs, preferences, routines, abilities, and NDIS goals.
Our focus is not simply on providing assistance—we support participants to take an active role in everyday life, make their own choices, develop practical skills, and enjoy greater independence at home and in the community.
ACTIVE SUPPORT
Building skills through everyday participation.
At MOC, we encourage participants to be actively involved in their daily routines and household activities wherever possible. Rather than doing everything for you, our support workers provide the right level of assistance, encouragement, and guidance to help you develop skills and confidence.
Support may include assistance with everyday routines, household tasks, meal preparation, personal organisation, and other activities that contribute to greater independence.
PERSONALISED SUPPORT
The right support, delivered around you.
Every participant has different needs, goals, preferences, and routines. Our personalised SIL support is designed around the individual rather than using a one-size-fits-all approach.
Our trained and experienced support workers take the time to understand how you prefer to be supported and work alongside you to achieve your goals while respecting your privacy, dignity, choices, and independence.
COMMUNITY ENGAGEMENT
Supporting connection, participation, and belonging.
Living independently also means having opportunities to participate in the community and maintain meaningful connections.
MOC supports participants to access local activities, attend appointments, pursue personal interests, build social connections, and take part in community life according to their individual preferences and goals. Our aim is to help participants feel more connected, confident, and included in the communities they choose to be part of.
PERSON-CENTRED APPROACH
Your choices and goals remain at the centre of your support.
At MOC, we focus on what matters most to you—your strengths, preferences, relationships, routines, aspirations, and personal goals.
You are encouraged to have choice and control over your daily life and the way your support is delivered. We work with you to create a supportive living environment that promotes dignity, safety, independence, wellbeing, and personal growth.
